Healthcare leaders are often asked to do something deceptively simple: adopt evidence-based practices that improve outcomes.
On paper, this seems straightforward. Research identifies interventions that work. Organizations adopt those interventions. Outcomes improve.
In reality, the path is rarely that linear.
Evidence-based practices do not automatically translate into changes in daily clinical behavior. Even well-designed interventions can fail to produce results if the workforce does not have the support needed to integrate them into real-world care.
This is where implementation expertise becomes essential.
Implementation involves more than training. It requires systems that support skill development, feedback loops that help practitioners refine their approach, and leadership strategies that ensure new practices are sustained over time.
Without these elements, even strong evidence-based practices can fade into the background of busy clinical environments.
Motivational Interviewing illustrates this dynamic well.
The research supporting MI spans decades and demonstrates improvements across a wide range of health outcomes, including chronic disease management, substance use treatment engagement, and preventive health behaviors.
Yet many organizations that introduce MI training see only modest results.
The difference is rarely the intervention itself.
More often, it is the level of attention given to implementation.
Organizations that invest in coaching, feedback, and measurement of practice tend to see far stronger and more sustained improvements than those that rely on training alone.
As rural health systems begin deploying RHTP resources, this lesson may prove particularly important.
Transformation is not simply about choosing the right strategies.
It is about building the capacity to implement those strategies well.
